Sara Mateer Clark, 49, of the 7300 Block Three Chopt Road, Henrico, was charged with manslaughter, DUI manslaughter, DWI and reckless driving Thursday.
Police say Clark was intoxicated when she crashed her car into a Henrico County kidney dialysis clinic July 9, 2012. One person died in the wreck, another was injured by debris.
Barbara A. Carney-Clarke, 61, died at VCU Medical Center from injuries sustained in the wreck, police said.
Sara Clark is set to appear in court Tuesday, December 11.
